Membership Feedback Form: This site has an on-line Feedback form where members can send input to OASN. The information required from the user is Name, State, and E-mail address. The information is sent to an OASN staff member via an automated e-mail message. The information is used by OASN staff only to determine the needs of the school nurse community. No information is shared with any third parties. Electronic Mailing Lists: This site collects your e-mail address when you subscribe to an OASN Internet Discussion List. Your address is used to enroll (subscribe) you in the list(s) of your choice and to enable you to receive any e-mail sent to the list(s). Your e-mail address is accessible to only members of the list(s) and the list administrator at the OASN office. This information is not shared with any third parties and does not include any other Personally Identifiable Information.
